There are lots of things that you can do to help us, help you.
When you are referred to the Child and Adolescent Mental Health Services (CAMHS) there is lots of information you can provide to help us. You will be given a form called the 'Strengths and Difficulties Questionnaire' (SDQ).
So we can help with whatever is troubling you, we need to find out more information about you. On the questionnaire you will be asked lots of different questions about what your strengths are and what you find difficult. You might enjoy helping people or you might be finding it difficult to get on with friends. What ever the issue by giving us more information, we are better equipped to help.
What would be really helpful before you come to your appointment, would be for you to complete the questionnaire as best you can. Don't worry if you don't know the answers any information you provide will be helpful.
Please bring the questionnaire with you on your first appointment and give it to the receptionist.
On your last appointment or after 6 months of receiving help, you will be given another 'Strength and Difficulties Questionnaire' to fill in so we can see how well you are doing.
As well as the questionnaire you will be given the opportunity to tell us whether or not we have helped you. This form is called the CHI-ESQ. Please complete the form and tell us what you liked and what we need to change.
Your views are really important to us and will help us make changes to improve the care you receive.
